Product Details

Make subcontractor management easy, consistent and compliant with a Subcontractor Onboarding Form.

This editable form captures essential information you need from new subcontractors before work begins. It is designed to ensure that your subbies, and each of their employees, have the appropriate licences and qualifications (e.g. construction white card, high risk work licences) before they are allowed to work on your site or your project. It also allows you to capture important contact information, including emergency contacts for each worker.

This helps your business meet its WHS legal obligations and manage the duty of care it shares with other businesses. It ensures all parties are aligned on safety, responsibilities and expectations from day one.

Who is it for?

This Subcontractor Onboarding Form is ideal for any businesses who use subcontractors as part of their work, particularly those in construction, manufacturing and other high-risk industries.

This form should be completed by each of the subcontractor’s employees.

When is it used?

This form is designed to be used after you have selected and prequalified your subcontractor, but before their employees begin work on site.

What’s included

  • Contact and emergency contact details
  • Disclosure of any pre-existing medical conditions that may impact work safety
  • Licence and qualifications
  • Acknowledgement of key workplace rights and responsibilities
  • Declaration of commitment to a safe, healthy, and productive work environment

Meeting your legal requirements

When you have subcontractors working on your site or project, you have a legal responsibility to ensure they are appropriately qualified and competent to do the job safely. This is part of your shared duty of care with other businesses.

For any kind of construction work, you must also ensure that each person working on site has a construction induction card, or ‘white card’.

Note

This Subcontractor Onboarding Form pairs well with our Subcontractor Prequalification Form to help you ensure subcontractors are properly assessed before onboarding their workers.
